Our case is the sixteenth reported case of malignant cylindroma in the world literature. Cylindromas most commonly occur on the scalp, head and neck as solitary or multiple tumors. They occur sporadically, are solitary and occasionally painful. Solitary cylindromas occur sporadically and typically are not inherited. Malignant cylindroma is a rare and poorly understood cutaneous malignancy. There is a paucity of literature on these lesions, with only a select number of case reports and limited guidelines on management. We have diagnosed a case of dermal cylindroma of scalp in an elderly female patient by fine needle aspiration cytology of the lesion and further confirmation was done by histopathology. Cytology consists of many clusters of small round to oval basaloid cells surrounded and attached with magenta coloured acellular hyaline material. Cylindromas are benign skin appendage tumors that commonly present as single, slow-growing lesions on the head and neck. Lesions often appear in conjunction with spiradenomas and trichoepitheliomas, they typically are 0.5-6.0 cm in size (although some can grow larger), and they affect females more than males. Cylindromas are rare benign tumors arising chiefly on the scalp.
